Nine Showcause Notices Issued To Air India For Five Safety Violations In Last Six Months: Aviation MoS Over Ahmedabad Crash

The Civil Aviation Ministry has issued nine showcause notices to Air India in connection with five separate safety violations over the past six months, Minister of State for Civil Aviation Murlidhar Mohol informed the Rajya Sabha on Monday (21 July). Authorities have completed enforcement action in one of the cases, reported Hindustan Times.
Crash Investigation Underway
Addressing questions about the 12 June crash of Air India flight AI-171 in Ahmedabad, the minister stated that the Director General of the Aircraft Accident Investigation Bureau (AAIB) ordered an investigation under Rule 11 of the Aircraft (Investigation of Accidents & Incidents) Rules, 2017.
“A preliminary report on the accident has been published by AAIB on 12th July, 2025 and is available on their website http://aaib.gov.in. The investigation is in progress to determine the probable cause(s)/contributory factor(s) leading to the accident,” he said.

The 12 June accident involved a London-bound Boeing 787-8 Dreamliner, which crashed shortly after takeoff from Ahmedabad, killing 241 of the 242 onboard, and several people on the ground.
According to the preliminary AAIB report, both engine fuel switches were cut off within seconds of takeoff. Voice recordings from the cockpit revealed one pilot asking, "Why did you cut off?" to which the other replied, "I didn't do so."
